Job Description
We are seeking an experienced Emergency Department Case Manager to join our healthcare team in Bel Air. In this critical role, you will coordinate patient care, manage resources, and ensure optimal outcomes for emergency department patients while maintaining efficient patient flow and appropriate utilization of healthcare services. - Conduct comprehensive patient assessments to determine appropriate level of care and resource needs
- Coordinate patient care plans with healthcare providers, families, and community resources
- Facilitate safe and timely patient discharges and transitions of care
- Monitor and manage length of stay and resource utilization
- Collaborate with insurance providers for authorizations and coverage determinations
- Ensure compliance with healthcare regulations and documentation requirements
- Identify and address barriers to care and patient needs
- Participate in quality improvement initiatives and care coordination meetings
- Provide education to patients and families regarding care plans and available resources
- Monitor and track patient outcomes and satisfaction metrics
Qualifications
- Bachelor's degree in Nursing (BSN) preferred
- Current Maryland RN license or Master's in Social Work License (LMSW/LCSW) required
- Certified Case Manager (CCM) certification preferred
- Basic Life Support (BLS) certification required
- Minimum 3 years Acute Care experience required, 5 years for Social Workers
- Demonstrated experience in emergency department or acute care setting
- Proficiency in Electronic Health Record (EHR) systems
- Strong knowledge of healthcare regulations, insurance processes, and compliance requirements
- Excellent interpersonal and communication skills
- Proven ability to work effectively in fast-paced environments
- Strong organizational and time management skills
- Demonstrated problem-solving and decision-making abilities
- Experience with utilization management and resource allocation preferred
- Knowledge of community resources and care coordination processes
